銅筋鐵肋的秘密基地

自從領了醫院的那隻手機後,
我自己的那隻手機就真的變成行動的outlook express,
用來瞧瞧未接來電或是收收簡訊.
只有醫院那隻手機才是24hr的,
值班的時候當然是不能關機,

關於皮蛋和鹹蛋的製作方式,
上次看了維京百科之後又忘了,今天想到,趕緊抄下來~~~

s/s of liver cirrhosis
來源是UpToDate. 查一下liver cirrhosis有很多有趣的sign勒
(也有很多可以拿來電人的名詞啊~~)
spider angioma: 
正常人也會有,懷孕者可能會增加;和estradiol/free testosterone ratio有關
不過spider angioma的數目在liver cirrhosis者的身上跟疾病嚴重程度有相關
越多則病人有esophageal varices的risk也越大
palmar erythema
跟sex hormone的代謝有關.發紅的手掌主要是在thenar and hypothenar area
其他情形,像是hyperthyroidism, hematogenous malignancy, pregnant RA也可見
hepatosplenomegaly
ascites
jaundice
Clubbing finger
缺氧, hypoalbuminemia有關
在肝硬化方面和biliary cirrhosis教有關
Hypertrophic osteoarthropathy (HOA)
long bone的chronic proliferative periostitis, caused pain
很多種的肝臟疾病都可能有此現象
Durpuytren's contracture
palmar fascia fibrosis尤其容易造成4th 5th手指成爪狀.
不過不是由liver cirrhosis造成的.
抽煙,喝酒,DM,reflex sympathetic dystrophy都是可能原因
Nail problem: 
    Terry's nail: 指甲proximal 2/3白 distal 1/3紅. hypoalbuminemia有關
    Muehrcke's nails: 指甲中間莫名其妙的一個transverse white line與hypoalbuminemia有關
                                     不是Beau's line
Gynecomastia
Cruveilhier-Baumgarten murmur
Caput medusa
Fetor hepaticus:
    因為dimethylsulphide增加,病人呼吸有刺激的味道
Asterixis:雙手outstretched wrist dorsiflexion, flapping; 見於hepatoencephalopathy者
Parotid gland enlargement: 跟alcohol有關(不是liver cirrhosis直接造成的)
